The helmet appears where the girl was laying because, since everyone sort of agrees that the boy was John, he was looking over at the girl in his flashback. That's from his POV. When he finally comes to, he's in the same position he was when he was a child, looking where the girl was, but his helmet is there instead.
This is why I believe he looks at his hand. He doesn't know he's a Spartan when he comes to, he has to reassure himself.
