CoD4 Extra Mission
I just saw this over at IGN.com along with the other cheats (well, not really cheats, but awards when you get the intel, etc).
Anyway - were you all aware there's another level after the credits roll? Check it out:

To play an all-new mission, which is an extra mission after the main quest is completed, beat the game and view the ending credits thereafter. After they've run their course, you will then be given the opportunity to play one last single player mission before all is said and done.

Yeah, you also get an achievement for it if you do it on Veteran. You can't skip the credits either, so you have no choice but to play it.

I quit out of the credits and was still able to play the "Mile High Club" extra mission.

I'm within 4 seconds of beating it. You do have to run past the first guy. Hang a left so he doesn't pop you though.
Tip:Never reload. Just pick up weapons on the ground.

I was trying for over an hour, on Veteran, and the closest I could manage was about 25m away... My dumbass team doesn't help me at all.

It's definitely rough, but it's doable. You can not do it without relying on your stun grenades though, they are vital.
Also, if you can get past the bad guys that are stunned without killing them, do it. You won't waste the ammo and your team mates will clean up behind you. They just don't clean up in front of you.
If you can grab the first shotgun, and reload it while crouching in front of the seats, it may be your best bet. It's what worked for me, but everyone has their own comfort zone.

Throw a stun grenade from the area that they give you more at into the final hallway. There should be tangos stunned in the narrow hallway and behind the chairs. Kill the ones behind the seats, and let your guys mop up the ones right in front of you in the hallway while you reload. Then you'll have shots for the two dudes around the corner right before the hostage standoff.
